After graduating with her Bachelor's of Science from Saint Mary's College of California in 2008, Dr. Brandy Tarap, MD married her husband, Paul, in 2009, and immediately began medical school. She attended Ross University School of Medicine where she spent a year and a half in Dominica, followed by two and a half years in Michigan, before graduating with her MD in 2013.
After an extensive interview process for residency, Brandy ranked Rapid City Residency Clinic as her number one choice after rotating there as a medical student. She then spent the next three years in Rapid City finishing her residency training and was chief resident her final year.
Her love of the Black Hills small towns had her decide to stay in Custer. Brandy has worked in the area for over ten years. This includes work as both the community's trauma and ambulance director.
Brandy and Paul have two daughters, Paisley and Emmy, who consume so much of their love and time. Both grew up raising cattle, so to carry on this tradition of instilling farm life responsibility, have begun raising cattle with their daughters. The current family farm also has goats, ponies, ducks and chickens running around for some extra family fun!
As she always enjoys being active, Brandy was an avid softball player and loves hiking the beautiful Black Hills with her family. She often can be found hunting or shooting her bow with her daughters. Both girls also enjoy fishing with their mom. Brandy has developed a love for gardening. She has a large, beautiful flower garden and delicious veggie garden.

Born and raised in Pierre, South Dakota, she has taken several meaningful detours throughout her life—from spending 27 years in Casper, Wyoming, to a brief island chapter in Maui, and even working 23,000 feet in the air as a critical care flight nurse. After 20 years in ICU and trauma nursing, she earned her Doctorate of Nursing Practice in 2024 and now serves as a Family Nurse Practitioner at SHFM.
She has been married to her college sweetheart, Brad, for 32 years. Together, they have raised five children—four of whom were homeschooled and are now thriving in college, while their youngest is a sophomore at Hill City High School. Their faith plays a central role in their lives, and they are active members of St. Rose of Lima Church.
She is especially passionate about natural women’s health, with a focus on fertility and hormonal care. Outside of caring for patients, she enjoys gardening, baking, and traveling—particularly to Italy and Germany during the Christmas season, where Christmas markets and mulled wine never lose their charm.

Her journey in healthcare has been shaped by years of bedside nursing, family life,
entrepreneurship, and a strong focus on listening, caring, education, and patient-centered decision-making.
Tiffany is a Family Nurse Practitioner Graduate with over a decade and a half of nursing experience across many areas of nursing including Oncology, Postpartum Care, Surgical and Recovery settings at a Pediatric Dental Surgery Office and running/ owning her own Aesthetic Med Spa.
She began her healthcare career as a Certified Nursing Assistant on the Oncology floor, where she spent seven years providing hands-on patient care before earning her Bachelor of Science in Nursing in 2012. She then worked as a Registered Nurse on the Oncology floor for two years before they expanded CCI, followed by nine and a half years in Postpartum nursing, supporting women and families during critical, and new life transitions. She later went on to open and operate an IV hydration and Medical Aesthetics practice for three years. In this role, she provided IV Hydration Therapy, Vitamin Injections, and Aesthetic Treatments. These experiences strengthened her commitment to individualized, patient-centered care and high clinical standards.
Tiffany earned her Master of Science in Nursing from South University in November 2025,
specializing in Family Health. She will provide comprehensive family healthcare while also helping expand aesthetic services within the practice.
Outside of work, Tiffany is married to a Retired Master Sergeant of the United States Air Force, who has been employed at Custer Hospital for the past 15 years. Both Tiffany and her husband were born and raised in Rapid City, and they are proud parents to three boys. Their deep roots in the Black Hills have shaped their strong connection to the community and commitment to family. Tiffany is eager to begin seeing patients and looks forward to serving individuals and families throughout the Southern Hills Family Medicine practice.
Paul has taken on the role as office manager for the clinic. He has been and will continue to perform all of the business operations behind the scenes. Paul has both a bachelor’s and master’s degree in education from Saint Mary's College of CA, but instead of education, has always been in business for himself.
Paul feels very lucky to work so closely with his wife. He enjoys seeing these amazing women compassionately care for the community of Custer. His only concern is that the time demand of this job could cut into his time in the Black Hills hunting, fishing, and working with his cattle!
